1. The Danger in the Gap
Many electrical incidents do not happen because of a lack of technical knowledge; they happen because of a breakdown in communication. The gap between the outgoing shift and the incoming shift is one of the most vulnerable periods in any industrial facility.
When complex troubleshooting stretches across multiple shifts, the incoming technician rarely has the complete mental model of what has been bypassed, tested, or temporarily altered.
2. Common Handover Failures
- The “Verbal Only” Handover: Passing on critical system states in the parking lot or break room without written documentation.
- Incomplete LOTO Transfer: Failing to properly transition personal locks to shift locks, leading to the incoming shift working under someone else’s assumed isolation.
- The “Almost Done” Trap: The outgoing technician says, “I just need to land these last two wires,” but forgets to mention that a jumper was placed in the control panel to bypass an interlock during testing.
3. Structuring a Safe Handover
A robust shift handover for electrical work should always include:
- The Physical Walkdown: Don’t just look at the permit paper. Walk the panel, walk the isolation points, and physically point out the current state of the equipment.
- The Status of Interlocks and Jumpers: Every single temporary jumper, forced bit in the PLC, or lifted wire must be documented and actively handed over.
- Formal LOTO Continuity: If the work is continuing, the incoming shift must apply their locks before the outgoing shift removes theirs, ensuring zero gap in isolation.
